I hereby give my consent for my child listed above to travel in a Grenville Baker Boys & Girls Club vehicle from the Middle/High School in Locust Valley and Robert Finley Middle School in Glen Cove to the Grenville Baker Boys & Girls Club, at 135 Forest Avenue, Locust Valley, NY 11560. There will be no charge for this service. There will be no transportation home; it is my responsibility to get my child home. An active membership is a prerequisite.

I acknowledge that this transportation is being provided Mondays through Fridays when the Club is in full operation. I am aware that there will be no service on vacation days or holidays and that it may be withdrawn at any time. Pickups will occur promptly, weather permitting, at dismissal of full days from the Locust Valley Middle/High School. I recognize that the vehicle will not wait for my child, if my child is late, and if under any circumstance my child does not board the vehicle, I will be responsible for my child’s whereabouts and hold the Club harmless from further responsibility and inquiry. I will notify the Club in advance (24 hours), by calling (516) 676-1460, ext. 216, if my child will not use the transportation on any day they have a seat reserved. If my child does not utilize their reserved transportation service without advanced notice for two or more days he/she will lose their reserved seat in the vehicle.

I will instruct my child to act in a manner that is cooperative and cordial to the driver and other passengers while in the vehicle. I understand that this service is a privilege being provided to our family, which can be immediately denied if, in the discretion of the Club’s staff, my child does not exhibit proper behavior or conduct. I have read and understand the guidelines provided me regarding this transportation service being offered and I had the opportunity to ask questions. As consideration for using this service, I will release the Grenville Baker Boys & Girls Club personnel, staff, board and volunteers from any and all liability in connection with the transportation services being provided.
